Abstract
L'invention concerne un système et un procédé automatisés de réduction du taux de réadmissions hospitalières de patients. Une suite complète d'applications (applis) est fournie pour des systèmes de prise en charge de gestion sanitaire en temps réel de la technique par l'intermédiaire de dispositifs mobiles en nuage à destination de prestataires de soins de santé, de payeurs et de médecins. Ces outils sont conçus pour des plates-formes mobiles des systèmes d'exploitation iOS et Android ainsi que pour des ordinateurs de bureau et des ordinateurs portables classiques fonctionnant sous Windows, Apple OSX et Chrome OS. Le système comprend une application (appli) de programme, un entrepôt de données conforme à la loi sur la transférabilité d'assurance maladie et la responsabilité (HIPAA), et une disponibilité d'informations en temps réel.
