Definitions

  • EHR Electronic Health Records
  • many health care providers are now using mobile tablet and laptop systems for the purposes of eliminating paperwork and making the information collection process more efficient.
  • This adoption of technology for EHR and the use of mobile devices and apps for gathering data have opened an opportunity for apps that will not only make the process of gathering data more efficient but also provide ready access to useful information and value-added solutions that will be available to hospitals, caregivers, and payers to improve patient care and lower the rate of avoidable, preventable, or unnecessary (APU) hospital readmissions.
  • APU avoidable, preventable, or unnecessary
  • a method for reducing hospital readmissions using embodiments of the inventive platform includes the steps of compiling a stored list of user inputted survey answers via a graphical user interface regarding a sociological and physical status of the patient, where the survey answers are recorded in a database with searchable variables.
  • the answers may be grouped into groups that include at least two of: access to transportation, nutrition, polypharmacy, and cohabitation.
  • FIG. 2H is a search for patient information screen 40B to initiate a new survey for a patient where input fields need to be filled in for last name (entry field 42), first name (entry field 44), record number (entry field 46), member ID (entry field 48), birthdate (entry field 52), gender selection (buttons 54), social security number (entry field 56), and personal identification number (PIN) (entry field 58).
  • Selection of the search button 60 initiates the search based on the information inputted.
  • Cancel button 50 stops the survey and may also clear the input fields.

  • FIG. 3E illustrates a meter 122A in screenshot 90E which indicates the level of risk for readmission based on the survey answers and findings as determined based on predictive algorithms and historical records. The meter is color coded based on the danger factors as a spectrum of colors. Table listing 124 provides the factors used to calculate the readmission risk. The individual factors in table 124 may be color coded to indicate a risk factor.
