Abstract
L'invention concerne des dispositifs et des procédés pour traverser des occlusions totales chroniques dans des lumières corporelles (par ex., vaisseaux sanguins, etc). Les dispositifs et procédés peuvent faire appel à l'introduction assistée par un fil guide d'un dispositif conçu pour traverser une occlusion totale chronique (CTO) et créer un passage dans le CTO pour un dispositif de traitement (par ex., un ballonnet d'angioplastie, une endoprothèse vasculaire, etc.). Des dispositifs de la présente invention peuvent comprendre un manchon extensible constitué de matériau qui se dilate en réponse à des changements de température. Dans d'autres dispositifs, un manchon extensible peut être utilisé dans lequel au moins deux parties cylindriques à verrouillage mutuel peuvent être conçues de sorte que les parties cylindriques présentent un premier diamètre plus petit auquel le manchon extensible peut être inséré dans un CTO. Le manchon extensible présente également un second diamètre plus important qui peut être obtenu par déplacement des deux parties cylindriques à verrouillage mutuel l'une par rapport à l'autre de sorte que le manchon extensible passe du premier diamètre au second diamètre lorsque le manchon extensible est placé dans le CTO. Selon d'autres aspects, un cathéter-guide peut comprendre une surface extérieure à frottement amélioré pour aider à maintenir le cathéter-guide dans une position sélectionnée lors de l'utilisation. La propriété de frottement amélioré peut être obtenue à l'aide, par ex., de matériaux présentant un coefficient de frottement accru, conférant des caractéristiques supplémentaires à la surface d'un cathéter-guide autrement lisse, etc.
