Abstract

Divers modes de réalisation de la présente invention concernent un procédé et un appareil destinés à améliorer le soin des patients (par exemple, des patients chroniquement malades). Dans un exemple (lequel devrait être illustrateur et non restrictif), l'invention devrait permettre d'améliorer ces soins par sortie en temps réel (par exemple, périodique, tel que par heure, par jour, par semaine ou par mois) afin d'empêcher le développement de comorbidités associées à plusieurs maladies chroniques. Divers modes de réalisation de cette invention peuvent améliorer les soins de patients chroniquement malades par: facilitation de la collecte de données, simplification de la transmission de données, interprétation efficace des informations de patients chroniquement malades, fourniture de séries chronologiques afin de former la base d'une analyse ultérieure, extension de la capacité du patient chroniquement malade ou d'un autre utilisateur de l'invention de comprendre la relation entre la pratique médicale et le style de vie propre au patient et/ou la facilitation de la charge de travail des soignants (notamment le médecin du patient). De ce point de vue, l'invention peut faciliter la création de dialogue, lequel serait autrement impossible, entre des patients chroniquement malades et des soignants.
