Abstract

L'implant fémoral (1) comporte : une tige fémorale (2) qui comprend une partie diaphysaire (4) destinée à s'étendre dans la diaphyse d'un fémur humain, et une partie métaphysaire (6) destinée à s'étendre dans la métaphyse du fémur ; un col (8) qui comprend une base (10) se raccordant à la partie métaphysaire (6) de la tige ; et une patte (20) solidaire de la base du col (16), disposée transversalement à la direction longitudinale de la partie métaphysaire (6) de la tige (2), s'étendant au moins sur deux des cotés interne, postérieur et externe du col (8) et dans laquelle sont ménagés des trous (24) adaptés pour recevoir chacun un ensemble amovible (25) de solidarisation de la patte au fémur, l'ensemble patte (20) ensemble de solidarisation (25) assurant la fixation de l'implant (1) sur le fémur.

Description

"Implant fémoral pour prothèse de hanche"
La présente invention concerne un implant fémoral pour prothèse de hanche, du type comportant une tige fémorale qui comprend une partie diaphysaire destinée à s'étendre dans la diaphyse d'un fémur humain, et une partie métaphysaire destinée à s'étendre dans la métaphyse du fémur ; un col de réception d'une tête fémorale qui comprend une base se raccordant à la partie métaphysaire de la tige fémorale ; et des moyens de fixation de l'implant sur le fémur.
Dans toute la suite, les termes "antérieur", "postérieur", "interne" et "externe" s'entendent dans leur sens anatomique, c'est-à-dire par rapport à un fémur du squelette humain.
Une prothèse de hanche comporte deux ensembles essentiels : un implant fémoral fixé sur l'os du fémur et un implant cotyloïdien fixé sur l'os iliaque. La coopération de ces deux implants est destinée à rendre à un patient muni de la prothèse de hanche, les mêmes capacités de mouvements et de déplacements que celles permises par une hanche naturelle. La pièce fémorale de la prothèse de hanche subit à la fois des contraintes en rotation, notamment lorsque le patient s'assoit, et des contraintes en compression qui tendent à enfoncer l'implant fémoral dans le fémur. Du fait du positionnement en porte-à-faux du col de l'implant par rapport à sa tige fémorale, ces contraintes provoquent un basculement.de l'implant. Aussi, la stabilité de l'implant par rapport à l'os se doit d'une part d'être
"primaire", c'est-à-dire que l'implant doit se positionner de façon adéquate par rapport au fémur lors de la pose de l'implant. La forme et la technique de pose de l'implant contribuent à la stabilité primaire.
La stabilité de l'implant se doit d'autre part d'être "secondaire", c'est-à-dire que l'implant doit demeurer en place après implantation, notamment pour les prothèses sans ciment par une réhabitation osseuse à partir de la structure osseuse du fémur. Pour améliorer la stabilité secondaire des prothèses sans ciment, il est connu par exemple de traiter la surface de la prothèse.
Afin d'assurer la fixation sans ciment de l'implant sur le fémur, divers moyens ont été ou sont actuellement utilisés. Pour assurer un blocage, en particulier en rotation, de la tige fémorale par rapport au fémur, on a envisagé plusieurs aménagements possibles :
- la partie diaphysaire de la tige de l'implant peut être de section sensiblement rectangulaire, assurant un blocage diaphysaire de l'implant par effet de coin ; - l'ancrage diaphysaire peut éventuellement être amélioré par verrouillage au moyen de clavettes s'étendant transversalement à la tige et traversant de part en part la diaphyse du fémur et la tige de l'implant ;
- la partie métaphysaire de la tige peut être de dimension élargie, de façon à augmenter la surface de contact entre la tige et la surface interne de l'alésage de la métaphyse ; et/ou
- un effet de collage entre l'os et l'implant peut être assuré par la pose d'un ciment entre la tige et le fémur.
Tous ces moyens de blocage se révèlent insuffisants en terme de tenue aux efforts de compression et surtout de rotation. Le blocage diaphysaire induit généralement un excès de contraintes préjudiciables dans le temps au fémur et entraîne des difficultés d'extraction de l'implant. Une grande surface de contact entre la tige et la métaphyse implique une grande destruction osseuse pour la mise en place de l'implant et fait courir le risque au patient d'une fracture du fémur en présence d'une inconformité entre la tige et la cavité métaphysaire du fémur.
Par ailleurs, pour améliorer la tenue, notamment primaire, aux forces de compression, certains implants ont été pourvus d'une collerette d'appui sur le bord interne de l'extrémité du fémur, au niveau de l'éperon de Merckel.
La présence de cette collerette est très souvent gênante puisque soit elle s'appuie sur l'éperon de Merckel avant que l'enfoncement de la tige fémorale ne soit terminé, empêchant sa bonne fixation dans la métaphyse, soit l'enfoncement est mené à bien sans que la collerette ne vienne en appui sur l'éperon de Merckel, rendant alors inutile la collerette.
Le but de l'invention est de proposer un implant fémoral du type précité, qui ne présente pas les inconvénients mentionnés ci-dessus, qui n'empêche pas la bonne congruence métaphysaire de la prothèse, et qui assure une très bonne retenue, tant en compression qu'en rotation, de l'implant par rapport au fémur.
A cet effet, l'invention a pour objet un implant fémoral du type défini plus haut, et dans lequel les moyens de fixation comportent une patte solidaire de la base du col, disposée transversalement à la direction longitudinale de la partie métaphysaire de la tige fémorale, s'étendant au moins sur deux des cotés interne, postérieur et externe du col et dans laquelle sont ménagés des trous adaptés pour recevoir chacun un ensemble amovible de solidarisation de la patte au fémur.
Suivant d'autres caractéristiques de cet implant, prises isolément ou selon toutes les combinaisons techniquement possibles : - la tige fémorale, le col et la patte de l'implant fémoral sont réalisés d'un seul tenant ;
- au moins un trou de réception d'un ensemble de solidarisation est ménagé dans chacune des parties interne, postérieure et externe de la patte ; - la base du col se raccorde sensiblement continûment à la partie métaphysaire de la tige fémorale au moins sur le côté antérieur du col ;
- les trous sont orientés suivant des directions obliques les unes par rapport aux autres ;
- les directions respectives d'au moins certains des trous de réception d'un ensemble de solidarisation sont sensiblement perpendiculaires à la face tournée vers l'extrémité libre du col de la patte ;
- l'épaisseur de la patte est sensiblement constante ;
- la face tournée vers la tige fémorale de la patte est sensiblement plane ;
- la surface tournée vers l'extrémité libre du col de la partie externe de la patte est inclinée par rapport à la face tournée vers la tige fémorale de la patte de façon que la direction du ou de chaque trou ménagé dans ladite partie extérieure est sensiblement perpendiculaire à ladite surface inclinée ;
- chaque ensemble de solidarisation comporte une tige d'ancrage de l'implant dans la matière osseuse du fémur, et des moyens de blocage de la tige d'ancrage par rapport à la patte ;
- les ensembles de solidarisation permettent une structure "pilotis" ;
- la tige d'ancrage est une vis comportant un corps fileté destiné à être ancré dans le fémur ; et
- les moyens de blocage comportent une contre-vis filetée adaptée pour coopérer avec un filetage associé ménagé dans la partie tournée vers l'extrémité libre du col du trou correspondant.
L'invention sera mieux comprise à la lecture de la description qui va suivre, donnée uniquement à titre d'exemple, et faite en se référant aux dessins sur lesquels :
- la figure 1 est une vue en perspective et en éclaté d'un implant fémoral selon l'invention ;
- la figure 2 est une vue de face selon la flèche 2 indiquée sur la figure 1 ;
- la figure 3 est une vue de face selon la flèche 3 indiquée sur la figure 1 , de l'implant de la figure 1 mis en place et fixé sur un fémur humain vu en section interne-externe ; et - la figure 4 est une vue en coupe selon le plan IV-IV indiqué sur la figure 3, d'une partie de l'implant fixé sur le fémur avec un ensemble amovible de solidarisation envisageable.
L'implant 1 représenté aux figures 1 et 2 comporte une tige fémorale 2 constituée d'une partie diaphysaire 4 adaptée pour s'étendre à l'intérieur du canal médullaire diaphysaire d'un fémur humain et d'une partie métaphysaire 6 adaptée pour s'étendre à l'intérieur du segment métaphysaire du fémur. Comme il est connu dans la technique, les parties diaphysaire et métaphysaire de la tige s'étendent dans des directions respectives différentes afin de respecter l'anatomie du fémur. L'implant 1 comporte un col 8 de fixation d'une tête fémorale non-représentée, telle qu'une tête articulaire sphérique destinée à coopérer avec un évidement de forme sensiblement complémentaire d'un implant cotiloïdien également non- représenté.
Plus précisément, le col 8 comporte, depuis la tige 2, une base 10 de forme tronconique, un cylindre 12 et un cône morse 14 de réception de la tête fémorale précitée. La base du col 10 se raccorde sensiblement continûment à la partie métaphysaire de la tige 6 suivant la face antérieure du col, comme plus visible sur la figure 2.
Le col 8 comporte une patte 20 s'étendant sensiblement perpendiculairement à la direction longitudinale de la partie métaphysaire de la tige 6. La patte 20 s'étend circonférentiellement autour de la base 10 suivant ses côtés interne, postérieur et externe, formant des parties de patte référencées respectivement 20A, 20B et 20C.
Cette patte est préférentiellement réalisée d'un seul tenant avec le reste de l'implant fémoral 1 , qui est constitué de tout matériau convenable, par exemple acier inoxydable ou titane, rendus au besoin réhabitables et revêtus ou non de céramique.
La patte 20 ménage de la sorte une face 21 tournée vers le cône morse 14 et une face 22 tournée vers la tige fémorale 2. Il est important de noter, comme il sera expliqué plus en détail lors de la description de la mise en place de l'implant 1 sur le fémur, que cette face 22 est destinée à demeurer à distance de l'os lors de la pose et de l'utilisation de l'implant.
La totalité de la face 22 est sensiblement plane, tandis que la face 21 est sensiblement parallèle à la face 22 pour les parties interne 20A et postérieure 20B de la patte, et est inclinée par rapport à la face 22 pour la partie externe 20C. Les parties 20A et 20B de la patte sont de la sorte d'épaisseur sensiblement constante et la partie 20C est renforcée par une plus grande épaisseur. Des trous traversants 24 sont ménagés dans la patte 20, suivant des directions obliques entre elles. Ces trous sont adaptés pour recevoir chacun un ensemble amovible 25 de solidarisation de l'implant fémoral 1 sur le fémur, comme mieux représenté sur la figure 3. L'étendue radiale, par rapport à l'axe du col, de la patte 20 est dimensionnée pour permettre la réalisation des trous 24 de réception des ensembles 25. L'exemple d'implant représenté comporte trois ensembles amovibles de solidarisation dont les trous correspondants 24 sont ménagés respectivement dans les parties interne 20A, postérieure 20B et externe 20C, de façon à assurer la fixation de l'implant par rapport au fémur sur ses côtés interne, postérieur et extérieur.
Dans l'exemple de réalisation représenté sur la figure 4, la partie tournée vers l'extrémité libre du col 8 de chaque trou 24 forme un logement fileté 26, et la partie destinée à être tournée vers le fémur de chaque trou forme un passage 28 débouchant sur la face 22 de la patte 20. La section transversale de ce passage 28 est plus petite que celle du logement 26 de sorte qu'un épaulement incliné 30 est formé dans le fond du logement.
De plus, chaque ensemble de solidarisation 25 comporte une tige, filetée ou non, qui est destinée à s'ancrer dans le fémur et qui est adaptée pour être bloquée dans la patte 20. Dans l'exemple de réalisation représenté, cette tige d'ancrage se présente sous la forme d'une vis 32 comportant un corps fileté 33 adapté pour être introduit dans le trou correspondant 24 et destiné à être ancré dans le fémur, suivant la direction imposée par le trou correspondant, et une tête 34 de section transversale plus grande que celle du corps 33 et adaptée pour s'appuyer contre l'épaulement 30 du trou 24. De plus, chaque ensemble de solidarisation 25 comporte une pièce 36 de blocage de la tige d'ancrage, en appui contre l'épaulement 30. Cette pièce de blocage 36 est, dans l'exemple de réalisation présenté, une contre-vis filetée coopérant avec le filet du logement 26 correspondant. La pièce de blocage 36 assure la solidarisation de chaque vis 32 à la patte 20 et impose la transmission des efforts entre le fémur et l'implant 1 par l'intermédiaire de l'ensemble patte 20-vis 32. Avantageusement, pour au moins certains des ensembles solidarisation 25, la face 22 de la patte 20 comporte, autour du trou correspondant 24, une zone dont l'inclinaison est sensiblement perpendiculaire à la direction d'ancrage de la vis 32 de l'ensemble de solidarisation, c'est-à-dire la direction du trou 24. Tel est le cas pour l'ensemble de solidarisation ancré dans la partie arrière de la métaphyse, ainsi que pour l'ensemble de solidarisation ancré dans la partie extérieure de la métaphyse par le choix adéquat de l'orientation de la zone de face 22 correspondant à la partie extérieure 20C de la patte en fonction de la direction de l'ensemble de solidarisation correspondant. La transmission d'efforts entre les vis et la patte 20 s'en trouve améliorée. L'utilisation de l'implant fémoral selon l'invention est la suivante :
L'extrémité supérieure du fémur est préparée de façon à recevoir la tige fémorale de l'implant 1 , par exemple au moyen de râpes métaphyso-diaphysaires. Le col du fémur est recoupé pour que la patte 20 demeure à distance du col et n'empêche pas l'enfoncement de l'implant. Une fois la préparation terminée, la tige fémorale 2 est introduite dans le canal médulaire du fémur jusqu'à ce qu'elle prenne une position anatomiquement correcte par rapport au fémur, avec une bonne congruence métaphysaire. La patte 20, ménagée sur la base du col 10 de l'implant, demeure à distance de l'extrémité supérieure du fémur en formant un jeu j_(figures 3 et 4) de par exemple quelques millimètres par rapport au fémur une fois l'implant stabilisé de façon primaire dans la métaphyse.
Puis les vis 32 sont introduites dans les trous 24 et sont ancrées dans la matière osseuse des cotés interne, postérieur et externe du fémur. Une fois l'os pénétré par les vis 32, ces dernières sont rendues solidaires de la patte 20 (par vissage de pièces de blocage 36 dans l'exemple représenté figure 4)
L'implant selon l'invention est ainsi solidarisé à l'os par des ancrages métaphysaires répartis sur les cotés interne, postérieur et externe du fémur, assurant la retenue en compression et en rotation de l'implant par rapport au fémur, tout en assurant la congruence de la prothèse à la métaphyse. La résistance à la compression est répartie sur toute la longueur des corps de vis 33, avantageusement orientés suivant les lignes de force s'établissant entre le fémur et la hanche lors de leurs mouvements respectifs. La tenue aux efforts rotatoires est obtenue par la pluralité de vis solidarisées 32 positionnées circonférentiellement au col 8 de l'implant 1 , notamment sur ses côtés interne et externe. L'implant selon l'invention ne requiert qu'une faible altération osseuse métaphysaire, limitant ainsi le risque de fracture, et peut être facilement retiré, en cas de nécessité, par dévissage et retrait des ensembles amovibles de solidarisation 25.
En variantes non-représentées, d'autres ensembles de solidarisation 25 de l'implant sur le fémur sont envisageables dans la mesure où ils permettent une solidarisation des vis à la patte 20 suivant une structure "pilotis", c'est-à-dire avec la face 22 de la patte 20 pouvant être distante de l'os, ou éventuellement en ménageant un contact sans pression avec cet os. Ainsi, des ensembles de solidarisation à contre-écrou, à bague expansive lors de la progression de la tête de vis, à logement cylindrique ou tronconique fileté ou non et coopérant avec une tête de vis filetée ou non, à blocage de la tête de vis par clavetage ou par une plaque vissée sur la patte 20, etc. sont envisageables sans sortir du cadre de l'invention.
A titre de variantes, la forme de la patte n'est pas limitée à celle décrite et représentée dans l'exemple développé ci-dessus. Ainsi, son épaisseur peut être constante, son profil peut être, au moins en partie, incurvé, son pourtour périphérique peut être plus arrondi, etc.

Claims

REVENDICATIONS
1. Implant fémoral pour prothèse de hanche, du type comportant une tige fémorale (2) qui comprend une partie diaphysaire (4) destinée à s'étendre dans la diaphyse d'un fémur humain, et une partie métaphysaire (6) destinée à s'étendre dans la métaphyse du fémur ; un col (8) de réception d'une tête fémorale qui comprend une base (10) se raccordant à la partie métaphysaire (6) de la tige fémorale (2) ; et des moyens de fixation de l'implant sur le fémur, caractérisé en ce que lesdits moyens de fixation comportent une patte (20) solidaire de la base du col (10), disposée transversalement à la direction longitudinale de la partie métaphysaire (6) de la tige fémorale (2), s'étendant au moins sur deux des cotés interne, postérieur et externe du col (8) et dans laquelle sont ménagés des trous (24) adaptés pour recevoir chacun un ensemble amovible (25) de solidarisation de la patte au fémur.
2. Implant suivant la revendication 1 , caractérisé en ce que la tige fémorale (2), le col (8) et la patte (20) de l'implant fémoral (1 ) sont réalisés d'un seul tenant.
3. Implant suivant l'une des revendications 1 ou 2, caractérisé en ce qu'au moins un trou (24) de réception d'un ensemble de solidarisation (25) est ménagé dans chacune des parties interne (20A), postérieure (20B) et externe (20C) de la patte (20).
4. Implant suivant l'une quelconque des revendications précédentes, caractérisé en ce que la base (10) du col se raccorde sensiblement continûment à la partie métaphysaire (6) de la tige fémorale (2) au moins sur le côté antérieur du col (8).
5. Implant suivant l'une quelconque des revendications précédentes, caractérisé en ce que les trous (24) sont orientés suivant des directions obliques les unes par rapport aux autres.
6. Implant suivant l'une quelconque des revendications précédentes, caractérisé en ce que les directions respectives d'au moins certains des trous (24) de réception d'un ensemble de solidarisation (25) sont sensiblement perpendiculaires à la face (21 ) tournée vers l'extrémité libre du col (8) de la patte (20).
7. Implant suivant l'une quelconque des revendications précédentes, caractérisé en ce que l'épaisseur de la patte (20) est sensiblement constante.
8. Implant suivant l'une quelconque des revendications précédentes, caractérisé en ce que la face (22) tournée vers la tige fémorale (2) de la patte (20) est sensiblement plane.
9. Implant suivant la revendication 8, caractérisé en ce que la surface tournée vers l'extrémité libre du col (8) de la partie externe (20C) de la patte (20) est inclinée par rapport à la face (22) tournée vers la tige fémorale (2) de la patte (20) de façon que la direction du ou de chaque trou (24) ménagé dans ladite partie extérieure (20C) est sensiblement perpendiculaire à ladite surface inclinée.
10. Implant suivant l'une quelconque des revendications précédentes, caractérisé en ce que chaque ensemble de solidarisation (25) comporte une tige (32) d'ancrage de l'implant (1 ) dans la matière osseuse du fémur, et des moyens (36) de blocage de la tige d'ancrage (32) par rapport à la patte (20).
11. Implant suivant la revendication 10, caractérisé en ce que les ensembles de solidarisation (25) permettent une structure "pilotis".
12. Implant suivant la revendication 10 ou 1 1 , caractérisé en ce que la tige d'ancrage est une vis (32) comportant un corps fileté (33) destiné à être ancré dans le fémur.
13. Implant suivant l'une quelconque des revendications 10 à 12, caractérisé en ce que les moyens de blocage (36) comportent une contre-vis filetée (36) adaptée pour coopérer avec un filetage associé ménagé dans la partie (26) tournée vers l'extrémité libre du col (8) du trou correspondant (24).
