- the instrument comprises a front portion designed as a tip or tapered part which can cooperate with the bone tissue and merges at the rear into a central portion, which in turn is connected to a rear portion.
- the known osteotomes cannot be used for forming holes far inside the mouth, i.e. at the back of the upper and lower jaws, which is actually the area of their main application because the jaw-bone tissue is generally more porous in the posterior areas.
- the difficulty involved in accessing the area far inside the mouth is caused by the design of the known instruments, where manual force first has to be used to apply the instrument tip to an exact position in the actual jawbone tissue, and a manual force is then exerted which is intended generally to form a hole extending straight up or down or substantially straight up or down in the jaw bone.
- the main object of the invention is to solve the set of problems mentioned above.
- the present invention is based on the realization that the instrument or osteotome must be provided with a hollowing and guiding mechanism which permits an exact application when forming holes in the jaw bone. Accordingly, the hollowing procedure must be able to be effected despite the said difficulty of access in conjunction with hole formation.
- the feature which can principally be regarded as characterizing an instrument according to the invention is that the front and/or central portion (s) of the instrument is (are) provided with one or more threads which are arranged in such a way that, as the instrument rotates relative to the bone tissue, they function in the bone tissue, during hole formation, essentially as a hollowing and guiding mechanism, and less as a threading mechanism.
- the said front portion has the shape of a cone or a truncated cone and the central portion has principally the shape of a cylinder. At least the greater parts of the front and central portions can be provided with one or more threads or thread parts.
- the thread or thread parts arranged on the first portion, shaped as a cone or truncated cone, is (are) also conical. Each thread or thread part is deigned as a relatively widely spaced thread. The bottom surface between two thread turns is substantially straight.
- the thread or thread part is designed with a salient thin thread profile.
- each thread or thread part above the body or core of the front and/or central portions, on which the thread/threads and thread parts are applied also assumes certain chosen values.
- the outer surface or peripheral surface of each thread or thread part is substantially straight.

The hole formation can thus be carried out sequentially, starting with an instrument of small calibre or diameter, and continuing with one or more larger instruments as hole formation proceeds .
- the thread in question has a structure which is essentially directed solely at a hollowing and guiding effect for hole formation, principally in porous bone tissue/jaw-bone tissue.
- the thread structure has thus been given a special precision which guarantees the said hollowing and guiding functions at the same time as it suppresses the thread formation in the tissue, which means that an almost optimum bone compression can be obtained and used in connection with the hole and that the risk of bone tissue being pushed away laterally from the hole wall can be avoided in a reliable manner.
- the attachment to the angled mouthpiece contributes to solving the accessibility problem, and hole formation can be effected with the desired speed of rotation of the instrument.
- an instrument which is basically known per se can be used, facilitating the use in connection with this type of hole formation.
- the hollowing force is built into the instrument function, no external force need be applied to the instrument, and instead the latter can be easily guided in the correct direction and thereafter effect the necessary insertion force during rotation of the instrument with the dental angled handpiece.

- the length L of the instrument is about 30 mm. Of this length, there is a total length LI of about 10 mm for the thread 4, 5.
- the cone-shaped portion and the thread on the latter have a length L2 of about 5 mm.
- the cylindrical portion lb has a core diameter Dl of about 3 mm, and the diameter at the end of the truncated cone-shaped front portion la has a diameter D2 of about 2 mm.
- the thread 4 follows the cone shape 6 of the said core part.
- the shape of the thread 5 follows the shape of the core part 7.
- the thread diameter D exceeds the body diameter Dl by about 0.5 mm.

- the values for the angle ⁇ lie in the range of 15 to 25°, preferably about 20°.
- the instrument must be rotated at a low speed, for example at a speed in the range of 5 to 20 rpm.
- the bottom 6', 7' of the thread can be said to be plane. All or some of the threads can be conical, and in the present case the thread 4 is conical and the thread 5 is cylindrical.
- the whole of the central portion is provided with a thread, likewise the whole of the cone-shaped portion.
- the osteotome or instrument is mounted in a conventional dental angled handpiece 2 and, by allowing the osteotome to slowly rotate downwards, a seat is created with an identical or similar structure to the previously disclosed manual osteotome.
- the difference is that with the osteotome according to the invention access is good and it is easy to guide it in the correct direction since it is not necessary to apply any force during the preparation or hole formation.
- Figure 3 also indicates a radius R at each thread's transition to the straight surface 7'. The said value on this thread can be about 0.25 mm.
