明細書 網膜色素上皮細胞の過剰増殖に係わる疾患の予防又は治療剤 [技術分野]
本発明は網膜色素上皮細胞の過剰増殖に係わる疾患の予防又は治療剤として 有用な医薬品組成物に関するものである。
さらに詳しく述べれば本発明は、 式
で表される N— ( 3， 4—ジメ トキシシンナモイル） アントラニル酸（一般名： トラニラスト） またはその薬理学的に許容される塩を有効成分として含有する ことを特徴とする網膜色素上皮細胞の過剰増殖に係わる疾患の予防又は治療剤 に関するものである。
本発明において網膜色素上皮細胞の過剰増殖に係わる疾患としては、 例えば、 増殖性硝子体網膜症などを挙げることができる。
[背景技術]
網膜色素上皮細胞の増殖に係わる疾患として知られている増殖性硝子体網膜 症は、 網膜剥離の治療方法として外科的に施行されている網膜復位術において、 その予後を悪化させる原因として問題視されている。 現在、 この増殖性硝子体 網膜症の治療方法は硝子体手術しかなく、 手術を施行しても視力予後は必ずし もよいとは言えないのが現状である。 この増殖性硝子体網膜症の発症およぴ進 展には網膜色素上皮細胞の増殖が重要な役割を果していることは周知であり、 多くの研究者により様々な薬剤の網膜色素上皮細胞に対する抑制効果力 s研究さ れている （眼科臨床医報， 9号， 1 8 8 6〜 1 8 9 0ページ （ 1 9 9 3年） ；
眼科臨床医報， 9号， 2 030〜 2 034ページ （1 993年） ） 。
一方、 硝子体手術の技術的進歩の結果、 網膜剥離の治癒率は著しく向上した。 しかしな力 sら、 その治癒率は 92〜94%が限度であると言われている。 その 原因として、 増殖性硝子体網膜症、 即ち、 網膜色素上皮細胞の増殖が大きく関 与しているものと考えられている。 それ故、 網膜剥離に対する網膜復位術の治 癒率の向上または増殖性硝子体網膜症の予防治療のために、 網膜色素上皮細胞 の過剰増殖に対して優れた抑制効果を有する薬剤の開発が大いに嘱望されてい トラニラス トは気管支喘息、 アレルギー性鼻炎、 ァトピー性皮膚炎およびァ レルギ一性結膜炎のアレルギー性疾患、 ケロイ ド ·肥厚性瘢痕の皮膚疾患の治 療剤として広く用いられている薬剤であり、 例えば、 アレルギー反応に起因す るケミカルメディェ一タ一 （C h em i c a l me d i t o r) の遊離を 抑制する作用、 皮膚組織における線維芽細胞のコラーゲン過剰産生を抑制する 作用、 冠動脈の血管平滑筋細胞の過剰増殖を抑制する作用を有することが知ら れている。
しかしながら、 トラニラスト力'網膜色素上皮細胞の増殖を抑制すること、 ま た、 増殖性硝子体網膜症の予防又は治療剤として有用であるということは全く 知られていない。
[発明の開示]
本発明は、 式
で表される N— (3， 4—ジメ トキシシンナモイル） アントラニル酸またはそ の薬理学的に許容される塩を有効成分として含有することを特徴とする網膜色 素上皮細胞の過剰増殖に係わる疾患の予防又は治療剤に関するものである。
本発明は、 前記式 （I ) で表される N— ( 3， 4—ジメ トキシシンナモイル） アントラニル酸またはその薬理学的に許容される塩を投与することによる網膜 色素上皮細胞の過剰増殖に係わる疾患の予防又は治療方法に関するものである。 本発明は、 網膜色素上皮細胞の過剰増殖に係わる疾患の予防又は治療用の製 剤の製造のための前記式 （I ) で表される N— ( 3， 4ージメ トキシシンナモ ィル） アントラニル酸またはその薬理学的に許容される塩の使用に関するもの である。
更にまた、 本発明は、 前記式 （I ) で表される N— （3， 4ージメ トキシシ ンナモイル） ァントラニル酸またはその薬理学的に許容される塩の網膜色素上 皮細胞の過剰増殖に係わる疾患の予防又は治療剤としての使用に関するもので ある。
本発明者らは網膜色素上皮細胞の過剰増殖に対して抑制効果を示す化合物を 見いだすべく鋭意研究した結果、 トラニラスト力網膜色素上皮細胞の増殖を顕 著に抑制する作用を有することを見出し、 網膜色素上皮細胞の過剰増殖に係わ る疾患の予防又は治療剤として極めて有用であるという知見を得、 本発明をな すに至った。
本発明者らは、 ラッ トの網膜色素上皮細胞を用いた i n V i t r 0の細胞 増殖阻害作用確認試験において、 トラニラス ト力顕著に網膜色素上皮細胞の増 殖を抑制することを確認した。
このように、 トラニラストは優れた網膜色素上皮細胞の増殖抑制効果を有す るものであり、 網膜色素上皮細胞の過剰増殖に係わる疾患の予防又は治療剤と して有用な化合物である。
従って、 トラニラストまたはその塩を有効成分として用いることにより、 網 膜色素上皮細胞の過剰増殖に係わる疾患の予防又は治療剤として有用な医薬品 組成物を製造することができる。
有効成分であるトラニラストおよびその塩の製法は種々知られており、 文献 記載の方法等により容易に製造することができる （例えば、 特公昭 5 6— 4 0 7 1 0号、 特公昭 5 7— 3 6 9 0 5号、 特公昭 5 8— 1 7 1 8 6号、 特公昭 5
8— 4 8 5 4 5号、 特公昭 5 8— 5 5 1 3 8号、 特公昭 5 8— 5 5 1 3 9号、 特公平 1— 2 8 0 1 3号、特公平 1一 5 0 2 1 9号、特公平 3— 3 7 5 3 9号）。
トラニラストの薬理学的に許容される塩としては、 例えば、 ナトリウム塩、 カリウム塩等の無機塩基との塩、 モルホリン、 ピぺリジン、 ピぺラジン、 ピロ リジン等の有機ァミン、 アミノ酸との塩を挙げることができる。
本発明の医薬品組成物を実際の治療に用いる場合、 経口的に投与してもよいが 、 点眼剤、 眼軟膏剤、 注射剤等による局所投与が好ましい。
点眼剤は、 例えば、 トラニラストまたはその薬理学的に許容される塩および 塩基性物質を、 界面活性剤を溶解した適量の滅菌精製水に加熱溶解し、 これに ポリビニルピロリ ドンを加え、 次いで必要に応じ、 保存剤、 安定化剤、 緩衝剤、 等張化剤、 酸化防止剤、 粘稠化剤などの医薬品添加物を適宜加えて完全に溶解 させることにより製造することができる。
例えば、 眼軟膏剤は、 一般的に眼軟膏剤に用いられる基剤を用いて適宜調製す ることができる。 また、 可逆性熱ゲル化水性医薬品として使用することもでき る。
例えば、 注射剤として用いる場合、 硝子体等の患部組織またはその隣接組織 中に細い注射針で直接注入してもよく、 また眼内灌流液として使用してもよい。 また、 本製剤は徐放性製剤として投与してもよい。 例えば、 担体として徐放 性ポリマーを用いて、 トラニラス トまたはその薬理学的に許容される塩をこれ ら徐放性ポリマーのペレッ トあるいはマイクロカプセルに取り込ませて、 この ペレツ トあるいはマイクロカプセルを治療すべき組織中に外科的に移植するこ とができる。 徐放性ポリマーと しては、 エチレンビニルアセテート、 ポリヒ ド ロメタクリ レー ト、 ポリアクリルアマイ ド、 ポリビニルピロリ ドン、 メチルセ ルロース、 乳酸ポリマー、 乳酸 · グリコ一ル酸コポリマー等を挙げることがで き、 好ましくは、 生分解性ポリマーである乳酸ポリマー、 乳酸 ' グリコール酸 コポリマー等を挙げることができる。
本発明の医薬品組成物を実際の治療に用いる場合、 その有効成分であるトラ 二ラストまたはその薬理学的に許容される塩の投与量は患者の年齢、 疾患およ
び治療の程度等により適宜決定され、 治癒効果が発揮できる適当な濃度に設定 すればよい力^ 例えば、 点眼の場合、 好ましくは 0. 00 1〜 2重量％に調製 した点眼剤を、 1日 1〜数回、 1回当たり 1〜数滴点眼する。 [発明を実施するための最良の形態]
本発明の内容を以下の実施例によりさらに詳細に説明する。
実施例
網膜色素上皮細胞の増殖阻害作用確認試験
( 1 ) 網膜色素上皮細胞の単離および培養
S a k a g a m i ら （Op h t h a l m i c R e s. ， V o l . 2 7, 2 62〜 26 7ページ （1 995年） ） の方法に準じてラッ トょ り網膜色素上皮 細胞を単離、 培養した。 即ち、 7日齢の B r o wn No rw a yラッ トをぺ ントバルビタールの腹腔内投与にて麻酔し、 眼球を摘出した。 眼球を 70%ェ タノールに浸漬して滅菌後、 H a n k s' b a l a n c e d s a l t s o l u t i o n (HB S S) で洗浄し、 0. 1 %プロティナーゼ K溶液中で 3 7°C、 1 5分間インキュベート した。 眼球を HB S Sで洗浄後、 1 0%ゥシ胎 児血清 （FB S ) を含有した D u l b e c c o ' s mo d i f i e d E a g 1 e ' s M e d i um (DMEM) と H a m， s F 1 2の 1対 1混合培 地に浮遊させた後、 鋸状縁に沿って切開を加えて前眼部、 水晶体及び硝子体を 取り除いた。 実体顕微鏡下で網膜より網膜色素細胞層を剥離し、 カルシウムお よびマグネシゥム不含の H B S Sで洗浄後、 0. 1 %トリプシン溶液中で 37 Τ 7分間インキュベートした。 ピペッティングによって細胞を単離し、 1 0%ゥ シ胎児血清 （F B S) を含有した DMEMと H am' s F 1 2の 1対 1混合 培地で継代培養して実験に供した。
(2) 供試薬物の調製
トラニラストをジメチルスルホキシド （DM SO) に溶解し、 最終濃度が所 定濃度になるように血管内皮細胞培養培地で希釈して調製した。 DMSOの最 終濃度は 0. 5%とした。
(3) 実験操作
コラーゲンコ一トした 9 6穴のプレートに各穴 4 X 1 0 4個の細胞浮遊液 2 0 0 μ I を添加し、 3 7°C、 5 %炭酸ガス— 9 5 %気相下で培養した。 翌日、 培養液を各種濃度のトラニラス トを含有し、 1 0 %FB Sを含有した DMEM と H am， s F 1 2の 1対 1混合培地 2 0 0 1に交換して培養した。 培養 4日後、 細胞増殖測定用試薬 A 1 ama r B l u eを各穴 2 0 1添加して、 更に 4時間培養し、 ィムノリーダーにて 6 2 0 nmの吸光度を測定し細胞数を 算出した。
(4) 効果判定
無処置群の細胞数を 1 0 0%とし、 各種濃度のトラニラスト処置群の細胞増 殖抑制率を求めた。
(5) 結果
以下の表に示されるように、 トラニラストは濃度依存的に網膜色素上皮細胞 の増殖を顕著に抑制した。
本発明のトラニラストまたはその薬理学的に許容される塩を有効成分として 含有する医薬品は、 顕著な網膜色素上皮細胞の増殖抑制作用を有しており、 増 殖性硝子体網膜症などの網膜色素上皮細胞の過剰増殖に係わる疾患の予防又は 治療剤として極めて好適である。
Claims
1 . 式
で表される N— ( 3， 4—ジメ トキシシンナモイル） アントラニル酸またはそ の薬理学的に許容される塩を有効成分として含有することを特徴とする網膜色 素上皮細胞の過剰増殖に係わる疾患の予防又は治療剤。
2 . 点眼剤又は眼軟膏剤の形態にある請求項 1記載の網膜色素上皮細胞の過剰 増殖に係わる疾患の予防又は治療剤。
3 . 式
で表される N— （3， 4—ジメ トキシシンナモイル） アントラニル酸またはそ の薬理学的に許容される塩を投与することによる網膜色素上皮細胞の過剰増殖 に係わる疾患の予防又は治療方法。
4 . 網膜色素上皮細胞の過剰増殖に係わる疾患の予防又は治療用の製剤の製 造のための、 式
で表される N_ (3， 4—ジメ トキシシンナモイル） アントラニル酸またはそ の薬理学的に許容される塩の使用。
