The prosthesis consists of a flexible cable (1) of which one end is hinged to a pivot (2) implanted in the condylus while its other end is attached to the tibia via a length-adjusting arrangement (5, 6, 7). Said prosthesis is particularly suitable for sportspersons with a torn cruciate ligament who are required to resume their normal activities promptly.
Description
The present invention is intended to replace a natural cruciate ligament of the human knee, the failure of which provokes a significant handicap and can generate osteoarthritis. There are currently two routes in cruciate ligament surgery.
If this technique. not a problem of biocompatibi- ity, i * 'ameure risks stretching and weakening of the tendon which is taken the implant.
If the fixation on the tibia does not pose big problems, the more delicate femoral fixation is oriented in two ways:
- dans un tunnel trans-osseux condylien, ce qui respecte l'isométrie mais pose des problèmes d'usure du tunnel et d'interférence avec le bord du condyle.- in a condylar transosseous tunnel, which respects the isometry but poses problems of wear of the tunnel and interference with the edge of the condyle.
Ces ligaments peuvent poser des problèmes de tolérance; de plus, la tenue au vieillissement en milieu synovial es\ mal connue.- in "over the top" where the isometry is no longer respected which can give limitations in flexion or extension. These ligaments can cause tolerance problems; moreover, the resistance to aging in a synovial environment is poorly understood.
By the present invention, it is proposed to remedy the main drawbacks of current solutions.
The femoral end of the compor ligament has a loop which is articulated around a pivot implanted in the condyle. This articulation makes it possible to avoid abrasion of the condyle and to maintain the isometric functioning of the knee.
The tibial end has a screw-nut system that adjusts the length of the ligament. The use of metallic materials minimizes the risk of allergy.
In the description which follows, reference is made to the appended drawings, in which: - Figures 1 and 2 show, in partial sections respectively in profile and in front, a knee in extension equipped with the device.
The prosthesis consists of a flexible cable (1), one end of which is fixed to the tibia and the other end of which is articulated around a pivot (2).
This pivot (2) is implanted in the condyle at a point close to the center of the natural implantation of the cruciate ligament, perpendicular to the sagittal plane of knee flexion. It consists of a threaded rod fully embedded in the bone, extended by a cylindrical part (10) and a head (11) which protrude into the intercondylar space.
The articulation of the cable (l) on the pivot (2) is effected by means of a pad (3) adjusted on the cylindrical part (10); the head (11) serves as an axial stop.
This bearing '(3) has the shape of a pulley with a single groove, an angular sector of which is hollowed out to allow radial mounting on the pivot (2).
Une boucle (8) formée par l'extrémité du cable s'accroche dans la gorge et y est retenue par un sertissage (12) .A loop (8) formed by the end of the cable hooks into the groove and is retained there by a crimp (12).
A shelving (14) of a blank of the pad (3) allows a misalignment of the cable (1) relative to the groove of the pad (3). A not shown magnet placed nearby, collects the products of wear and tear from the joint and thus prevents their circulation in the body.
A threaded tube (6) is screwed from below, in a tibial tunnel opening into the implantation surface of the natural ligament and ensures the guidance of the cable (1) as well as good bone attachment.
L'utilisation d'un cable métallique permet de sertir sur l'extrémité un embout taraudé ou fileté (5).A protective tube (4), one end of which is curved and beveled, is placed in the tibial tunnel and in the tube (6) from above at the same time as the cable (1) on which it is threaded. It ensures during flexion of the knee a regular winding of the cable (1) around the curvature (13) without risk of bone wear. The use of a metal cable makes it possible to crimp a threaded or threaded end piece (5) on the end.
La longueur du ligament est facilement ajustée à l'aide de la vis (7) vissée dans l'embout (5) et prenant appui sur la face inférieure du tube (6) .The length of the ligament is easily adjusted using the screw (7) screwed into the tip (5) and bearing on the underside of the tube (6).
The adjusting screw (7) has a small section area (9) which must, during a very violent impact, break before the bone and before the cable. This screw is flush with the skin, it is easily replaceable.
The placement of this prosthesis is no more traumatic than that of existing prostheses. The work of the surgeon is facilitated by the possibility of correcting the length of the ligament during installation, or after a period of use. The femoral attachment detaches the cable from the condyle well which eliminates interference problems. The use of high performance metallic materials will allow long lifetimes and suggests good results when it comes to biological tolerance and aging in synovial medium.
