Abstract

Device allowing the remote transport of a needle mounted on a surgical thread and the making of surgical knots at its extremity. The device is comprised of a fixed handle (5) and a movable handle (1) which, acting on a serrated wheel (7), rotates by reaction an overlying wheel (10) which advances a cable or pinions (11) to which are hooked magnetized transport cabins (24-29 and 30-35) as well as a knot making system (18, 23). These cabins (24-35) are used to transport in both directions a needle (35) on which is mounted a surgical thread (43). The system (18-23) is used to transport at the extremity (41) of the fixed part of the device (15) the knot (42) which has been made outside and allows its tightening by spacing apart the branches (19, 20) of the system (18-23) on the stop (40).

Description

INSTRUMENT CHIRURGICAL POUR SUTURES OU LIGATURES PROFONDES SURGICAL INSTRUMENT FOR DEEP SUTURES OR LIGATIONS
L'un des reproches faits à la chirurgie percoelioscopique consiste en l'utilisation de laser ou de bistouri électrique pour couper ou assurer l'hémostase : ceci crée une source de chaleur dans un milieu péritonéal clos, ce qui entraîne parfois des lésions organiques (notamment digestives) directes ou par chaleur radiée, avec le problème des chutes d'escarres quelques jours plus tard entraînant des péritonites par exemple. Le dispositif décrit dans le présent brevet se propose de répondre en partie à ce problème en permettant le transport aller et retour d'une aiguille montée sur un fil chirurgical, puis la fabrication de nœuds pour assurer une ligature ou une hémostase correcte sans cautérisation. De plus amples détails sur le fonctionnement dudit dispositif apparaîtront au cours de la description qui va suivre (dessins donnés uniquement à titre d'exemple) .One of the criticisms made of percoelioscopic surgery consists in the use of laser or electrosurgical unit to cut or ensure hemostasis: this creates a source of heat in a closed peritoneal environment, which sometimes leads to organic lesions (in particular digestive) direct or by radiant heat, with the problem of pressure ulcers falling a few days later leading to peritonitis for example. Ces axes (19,20) sont manufacturés soit en un métal à mémoire de forme, soit reliées par un ressort de traction ou autre système de rappel (23) . Les pignons ou le cable de transport (11) sont reliés également à deux cabines de transport (24,25) de l'aiguille (45) montée sur fil chirurgical.Les deux cabines (24,25) sont aimantées,retenant l'aiguille (45), et sont légèrement séparées : l'aiguille (45) posée sur ces deux cabines (24,25) sera aisément prehensible au bout de son chenal de transport (37) par prise au milieu des deux cabines (24,25) . Chaque cabine (24,25) est munie d'un couvercle (28,29) se fermant automatiquement à l'entrée dans le canal (37) et s'ouvrant à sa sortie grâce à un ressort (28,29) .
