Abstract
Instrumento quirúrgico (11) que comprende: una parte de cuerpo (58); un elemento de herramienta (52) soportado sobre el extremo distal de la parte de cuerpo; y un recubrimiento alargado (10) soportado sobre el instrumento en una primera posición en la que el elemento de herramienta está descubierto, pudiendo desplazarse el recubrimiento desde la primera posición hasta una segunda posición que rodea por lo menos parcialmente al elemento de herramienta; un dispositivo de despliegue de recubrimiento (14, 116) asociado funcionalmente con el recubrimiento, pudiendo desplazarse el dispositivo de despliegue de recubrimiento para desplazar el recubrimiento desde la primera posición hasta la segunda posición; en el que el recubrimiento incluye un extremo proximal y un extremo distal y define un lumen (12), estando sujeto el extremo distal del recubrimiento al instrumento quirúrgico sobre el elemento de herramienta o junto a éste de tal manera que el lumen del recubrimiento se posicione alrededor de la parte de cuerpo y del dispositivo de despliegue de recubrimiento, invirtiendo el movimiento del dispositivo de despliegue de recubrimiento desde una posición retraída hasta una posición avanzada invierte el recubrimiento.

- Campo técnico
La presente solicitud se refiere a un recubrimiento a un dispositivo de despliegue de dicho recubrimiento para el elemento de herramienta o el conjunto de herramienta de un instrumento quirúrgico y, más particularmente, a un recubrimiento a un dispositivo de despliegue de dicho recubrimiento para el elemento de herramienta de un instrumento quirúrgico laparoscópico o endoscópico para manipular, tratar o extirpar tejido de una cavidad corporal, en particular tejido enfermo o canceroso de la misma.

- Antecedentes de la técnica relacionada
Se han desarrollado una variedad de diferentes tipos de instrumentos quirúrgicos para manipular, identificar, tratar, reparar y/o extirpar tejido, incluyendo órganos o partes de los mismos localizados dentro de cavidad corporales, denominándose tales instrumentos algunas veces en lo que sigue instrumentos quirúrgicos para el tratamiento de tejidos corporales. Estos instrumentos incluyen prensores, sujetadores, por ejemplo, grapadoras, disectores, dispositivos de biopsia, coaguladores, etc. Típicamente, estos instrumentos están dimensionados para utilizarse tanto en intervenciones abiertas como en intervenciones laparoscópicas o endoscópicas.
En intervenciones quirúrgicas endoscópicas para el tratamiento de tejido corporal, se inserta un instrumento quirúrgico a través de una incisión o cánula hasta una posición adyacente al tejido que se va a tratar. El elemento de herramienta distal del instrumento se manipula a continuación para tratar el tejido, es decir, para biopsiar, extirpar, diseccionar, coagular, reposicionar, etc., el mismo. A continuación, el instrumento con o sin tejido extirpado se extrae del sitio quirúrgico a través de la incisión para retirar el instrumento de la cavidad corporal.
Un problema asociado a los dispositivos quirúrgicos actuales es que, durante la retirada del elemento de herramienta del sitio quirúrgico, el elemento de herramienta o el conjunto de herramienta (en lo que sigue y por simplicidad “elemento de herramienta”) del instrumento quirúrgico para tratar, por ejemplo, tejido enfermo entra en contacto con el tejido sano que define la incisión y/o dentro de la cavidad corporal en la que está localizado el sitio quirúrgico. Esto puede ser un problema en intervenciones quirúrgicas abiertas. Cuando el tejido que se está tratando es tejido enfermo, por ejemplo canceroso, este problema llega a ser crítico, puesto que el contacto entre el tejido canceroso y el tejido sano puede dar como resultado una siembra de células cancerosas que a su vez se traduce en una metástasis.
La patente US nº 6.383.195 describe un aparato de retirada de muestras que comprende un conjunto de jaula/bolsa que lleva tiras cargadas por resorte que pueden juntarse para configurar una forma que se asemeja a un tulipán. El documento US 2003/0139767 describe una bolsa de recuperación de pared delgada similar a una funda que tiene tiras de recepción dispuestas en un extremo distal de la funda para permitir la recepción de una mordaza prensora. La bolsa se evierte sobre el tejido una vez que las puntas de recepción han prendido el tejido por el uso de un segundo dispositivo prensor.
Las mordazas prensoras pueden extenderse distalmente más allá del extremo distal de la bolsa de recuperación para permitirles directamente que el tejido sea agarrado directamente y recuperado por las propias mordazas, y a continuación, sea tragado por una bolsa evertida (figura 5a, §44).
En consecuencia, existe una necesidad en la técnica de un instrumental mejorado para proteger el tejido sano frente al tejido enfermo durante la retirada de un instrumento quirúrgico desde un sitio quirúrgico.
La presente descripción proporciona un recubrimiento de un elemento de herramienta para su utilización con un instrumento quirúrgico que tiene un conjunto de herramienta y una parte de cuerpo. El recubrimiento es preferentemente tubular y tiene unos extremos abiertos distal y proximal. Una parte del recubrimiento, el extremo distal, se sujeta al instrumento quirúrgico en posición adyacente al conjunto de herramienta o sobre el mismo. El recubrimiento puede sujetarse de forma amovible al instrumento quirúrgico. El recubrimiento es móvil desde una primera posición, en la que el elemento de herramienta está descubierto, hasta una segunda posición en la que el recubrimiento rodea por lo menos parcialmente el elemento de herramienta. Se proporciona un dispositivo de despliegue para mover el recubrimiento desde la primera posición hasta la segunda posición. El recubrimiento define un lumen posicionado alrededor de la parte de cuerpo y el dispositivo de despliegue de dicho recubrimiento. El movimiento del dispositivo de despliegue del recubrimiento desde una posición retraída hasta una posición avanzada invierte el recubrimiento.
Preferentemente, el recubrimiento está formado por un material impermeable. En una primera forma de realización, el dispositivo de despliegue incluye un manguito formado de semisecciones que se sujetan una a otra utilizando elementos expandibles, por ejemplo juntas tóricas elásticas. Las juntas tóricas permiten que las semisecciones del manguito se muevan hacia fuera una con relación a otra para permitir que el manguito pase sobre un conjunto de herramienta que tiene un diámetro mayor que una parte de cuerpo del instrumento quirúrgico. El manguito se posiciona de manera deslizante alrededor del cuerpo de un instrumento quirúrgico entre las posiciones retraída y avanzada a fin de mover el recubrimiento desde la primera posición hasta la segunda posición. El recubrimiento se posiciona alrededor de la parte de cuerpo del instrumento quirúrgico y alrededor del dispositivo de despliegue de tal manera que, cuando el dispositivo de despliegue se mueve de la posición retraída a la posición avanzada, el extremo distal del manguito se acople al extremo distal del recubrimiento para invertir el recubrimiento sobre el conjunto de herramienta.
El recubrimiento puede incluir un dispositivo de cierre, tal como un cordón de tracción o una banda elástica, para cerrar el extremo distal del recubrimiento a fin de encerrar el elemento de herramienta dentro del recubrimiento. El recubrimiento puede utilizarse con una variedad de diferentes tipos de instrumentos quirúrgicos, incluyendo grapadoras, sujetadores, manipuladores, dispositivos de biopsia, retractores, coaguladores, disectores, etc.
En la presente memoria, se describen diversas formas de realización preferidas del recubrimiento de elemento de recubrimiento y del dispositivo de despliegue de recubrimiento actualmente descritos haciendo referencia a los dibujos, en los que:

Las formas de realización preferidas del recubrimiento de elemento de herramienta y del dispositivo de recubrimiento actualmente descritos se describirán a continuación en detalle haciendo referencia a los dibujos, en los que números de referencia iguales designan elementos idénticos o correspondientes en cada una de las diversas vistas.
La figura 1 ilustra una forma de realización preferida del recubrimiento de elemento de herramienta y del dispositivo de recubrimiento actualmente descritos. Brevemente, el recubrimiento 10 de elemento de herramienta incluye un material flexible aplastable que es preferentemente impermeable a los líquidos y se forma a partir de una o más capas de un plástico adecuado, preferentemente polietileno. Alternativamente, otros materiales impermeables a los líquidos, tejidos o no tejidos, naturales o sintéticos y adecuados para su utilización quirúrgico pueden utilizarse para construir el recubrimiento, por ejemplo caucho, elastómeros politetrafluoretileno, etc. El recubrimiento 10 es preferentemente tubular y define un lumen 12 dimensionado o configurado para recibir la parte de cuerpo 58 del instrumento quirúrgico 11. Preferentemente, pero no necesariamente, el extremo distal 10a del recubrimiento 10 tiene un diámetro menor que el extremo proximal del recubrimiento 10. Una parte del recubrimiento 10 está sujeta al instrumento quirúrgico. El extremo distal 10a del recubrimiento 10 está dimensionado o configurado para sujetarse directamente sobre o junto a un elemento de herramienta 52 o un vástago que linda con un elemento de herramienta y un mango de un instrumento quirúrgico 11. Alternativamente, el recubrimiento 10 puede tener un diámetro constante a lo largo de toda su longitud o el extremo proximal del recubrimiento 10 puede tener un diámetro más pequeño que el extremo distal del recubrimiento 10. El recubrimiento 10 está sujeto preferentemente al instrumento quirúrgico de una manera impermeable a los líquidos. El recubrimiento 10 puede sujetarse también de manera retirable al instrumento quirúrgico.
Haciendo referencia también a las figuras 2 a 4, el dispositivo de despliegue de recubrimiento 14 (figura 5) incluye un manguito 16 formado por unas semisecciones de manguito 16a y 16b. La semisección de manguito 16a incluye un par de extensiones 18 que están dimensionadas para ser recibidas de manera deslizante dentro de ranuras 20 formadas en el manguito 16b. Cada semisección de manguito 16a y 16b incluye un extremo proximal 22 que tiene una brida o junta semianular 24, un rebaje semianular centralmente posicionado 25, un rebaje semianular distalmente posicionado 26 y un rebaje semianular proximalmente posicionado 27. El extremo distal 28 de cada manguito 16a y 16b incluye un saliente romo semianular 30. La función del saliente 30 se discutirá en detalle a continuación.
Haciendo referencia a las figuras 2 a 4, las semisecciones de manguito 16a y 16b están posicionadas en relación de apoyo a tope con las extensiones 18 posicionadas de manera deslizante en las ranuras 20 para definir un lumen 32 dimensionado con el fin de recibir de manera deslizante o posicionarse alrededor de una parte del cuerpo del instrumento quirúrgico 11. Un elemento expandible, preferentemente una junta tórica elástica 34 (figura 1), está posicionado en cada uno de los rebajes anulares 25, 26 y 27. Las juntas tóricas 34 pueden estirarse desde una posición contraída hasta una posición expandida con el fin de permitir que las semisecciones de manguito 16a y 16b se muevan una con relación a otra entre posiciones contraída y expandida (figuras 3, 4 y 9, respectivamente). En la posición contraída, los manguitos 16a y 16b se mantienen preferentemente en relación de apoyo a tope con las extensiones 18 completamente insertadas dentro de las ranuras 20.
Haciendo referencia a la figura 5, las semisecciones de manguito 16a y 16b están configuradas para ensamblarse fácilmente alrededor de la parte de cuerpo alargada 58 del instrumento quirúrgica 11, de tal manera que el extremo distal 28 del manguito 16 se posicione al lado de un conjunto de herramienta 52 del instrumento quirúrgico 11. Aunque el instrumento quirúrgico 11 se ilustra como una grapadora circular, se contempla que el instrumento quirúrgico pueda ser cualquiera de una amplia variedad de instrumentos para realizar una amplia variedad de funciones, incluyendo corte, sujeción, manipulación, tratamiento, disección, coagulación y biopsia de tejidos. Tales instrumentos incluyen retractores, dispositivos de biopsia, disectores, coaguladores, sujetadores, grapadoras, incluyendo grapadoras lineales, grapadoras circulares, grapadoras semicirculares, grapadoras en forma de arco, etc.
Haciendo referencia también a las figuras 6 y 7, en la forma de realización ilustrada la grapadora circular 11 incluye un conjunto de herramienta no aproximado 52 que tiene un yunque 54, un conjunto de envuelta 56 y un cuerpo alargado 58. En la solicitud PCT nº de serie PCT/US02/10792 se describe una grapadora circular conocida. El manguito 16 se posiciona de forma expandible y deslizable alrededor del cuerpo alargado 58 de tal manera que el saliente 30 se posicione al lado del conjunto de envuelta 56. El extremo distal 10a del recubrimiento 10 se asegura preferentemente junto a la parte proximal del conjunto de herramienta 52 o sobre ésta en un punto de sujeción o afianzamiento 60 (figura 7) utilizando una técnica de sujeción conocida, por ejemplo adhesión, fusión, recalcado, espigas, pinzas, etc., y se posiciona alrededor del manguito 16 y la parte de cuerpo 58 del instrumento quirúrgico 11. Alternativamente, el recubrimiento 10 puede asegurarse de forma liberable al instrumento utilizando técnicas conocidas, incluyendo resortes, lengüetas, ganchos, etc. El saliente anular 30 formado en el extremo distal 28 del manguito 16 se posiciona proximalmente con respecto al punto de sujeción o afianzamiento 60.
Se entiende que el recubrimiento 10 puede asegurarse en cualquier ubicación adecuada a lo largo del vástago 58, aunque en algunos instrumentos el afianzamiento de la misma cerca o incluso en el propio elemento de herramienta ahorrará material de recubrimiento y hará que el recubrimiento sea más fácil de manipular con o sin tejido en el mismo.
Haciendo referencia a las figuras 8 a 10, en la realización ilustrada el extremo distal 10a del recubrimiento 10 se sujeta a la parte proximal del conjunto de envuelta 56 del conjunto de herramienta 52. En uso, el instrumento quirúrgico 11 se posiciona de tal manera que el conjunto de herramienta 52 esté situado junto al sitio quirúrgico. Durante una intervención quirúrgica endoscópica, esto incluiría insertar el conjunto de herramienta 52 y una parte de la parte de cuerpo 58 a través de una incisión con el recubrimiento 10 y el dispositivo de despliegue de recubrimiento 14 soportados alrededor de la parte de cuerpo alargada 58 del instrumento quirúrgico 11. A continuación, se manipula el instrumento quirúrgico para que realice su función asociada o designada. Con respecto a una grapadora circular, el tejido corporal (no mostrado) se posiciona entre un yunque no aproximado 54 y el conjunto de envuelta 56 utilizando, por ejemplo, una sutura de fruncido (no mostrada). A continuación, el yunque 54 y el conjunto de envuelta 56 se aproximan y se activa el instrumento quirúrgico 11 de una manera conocida para diseccionar tejido y aplicar una agrupación ordenada circular de grapas al tejido diseccionado y restante. Después de que el instrumento quirúrgico 11 haya realizado su función asociada, pero antes de la retirada del conjunto de herramienta 52 del sitio quirúrgico, se empuja la junta anular 24 del manguito 16 hacia delante de manera manual, mecánica, remota o robótica en la dirección indicada por las flechas “A” en la figura 9 para hacer avanzar el manguito 16 alrededor del cuerpo 58 del instrumento quirúrgico 11. A medida que se hace avanzar el manguito 16, el saliente angular 30 se acopla al extremo distal 10a para invertir el recubrimiento 10 alrededor del punto de afianzamiento 60 y gradualmente se aplica alrededor del recubrimiento 10 en posición proximal respecto del conjunto de herramienta asegurado o sujeto 52. El saliente anular 30 incluye una superficie lisa o roma para impedir el desgarre del recubrimiento 10 durante el avance. Debido a que el conjunto de herramienta 52 tiene un diámetro mayor que el cuerpo 58, por lo menos las partes extremas distales de las semisecciones de manguito 16a y 16b se expanden hacia fuera una con relación a otra estirando las juntas tóricas 34 cuando el extremo distal 28 del manguito 16 corre sobre el conjunto de herramienta 58. Se observa que, en instrumentos quirúrgicos que tienen un conjunto de herramienta con un diámetro igual o menor que el diámetro del cuerpo del instrumento quirúrgico, no se requerirían las juntas tóricas estirables. Cuando se ha hecho avanzar el manguito 16 para desplegar completamente el recubrimiento 10 alrededor y preferentemente más allá del conjunto de herramienta 52 (figura 10), el dispositivo de despliegue de recubrimiento puede ser extraído hasta su posición retraída alrededor del cuerpo 58 del instrumento quirúrgico 11. El instrumento quirúrgico 11, incluyendo el conjunto de herramienta 52 y el tejido incisionado contenido en él o alrededor del mismo (no mostrado) y encerrado por el recubrimiento 10, puede ser extraído ahora del sitio quirúrgico sin exponer el tejido sano que define la incisión y dentro de la cavidad del cuerpo (no mostrada) a tejido enfermo presente sobre el conjunto de herramienta o dentro de éste.
Las figuras 11 a 13 ilustran otra forma de realización preferida del dispositivo de despliegue de recubrimiento actualmente descrito mostrado en general como 116. Se observa que los dispositivos de despliegue descritos aquí son sólo ejemplos de formas de realización. Cualquier dispositivo de despliegue adecuado capaz de posicionar un recubrimiento sobre un conjunto de herramienta de un instrumento quirúrgico después de que se le haya utilizado en una intervención quirúrgica está dentro del alcance de esta descripción. El dispositivo de despliegue de recubrimiento 116 incluye una parte de cuerpo semirrígido 122 que tiene una parte de guiado semianular proximal 124 y un elemento de acoplamiento semianular distal 126. El elemento de acoplamiento semianular 126 está formado preferentemente de un material suficientemente elástico en sentido diametral y radial y está configurado y dimensionado para acoplarse al extremo proximal de un conjunto de herramienta 152 de un instrumento quirúrgico
111. La parte de guiado proximal 124 está configurada para abarcar parcialmente a la parte de cuerpo 158 del instrumento quirúrgico 111 y para ser empujada por un cirujano a fin de hacer que funcione el dispositivo de despliegue 116. La parte de cuerpo 122 del dispositivo de despliegue 116 interconecta la parte de guiado 124 y el elemento de acoplamiento 126 y es suficientemente rígida como para transferir la fuerza de empuje impartida al elemento de acoplamiento 126.
El extremo distal 110a del recubrimiento 110 está asegurado al instrumento quirúrgico 111 junto a una parte proximal del conjunto de herramienta 152 o sobre ésta de la manera descrita anteriormente con respecto al recubrimiento 10, de tal forma que el recubrimiento 110 se posicione sobre el dispositivo de despliegue 116 y el cuerpo 158 del instrumento quirúrgico 111. El elemento de acoplamiento 126 se posiciona ligeramente proximal con respecto a un punto de sujeción 160 del recubrimiento 110 y al conjunto de herramienta 152. En uso, el dispositivo de despliegue 116 se hace avanzar alrededor del cuerpo 158 del instrumento quirúrgico 111 empujando sobre la parte de guiado 124 para hacer avanzar el elemento de acoplamiento 126 hacia el extremo distal del recubrimiento 110 para invertir el recubrimiento 110 alrededor del conjunto de herramienta 152 de una manera similar a la descrita anteriormente. Debido a que el elemento de acoplamiento 126 es flexible, es capaz de deslizarse sobre el diámetro creciente del conjunto de herramienta 152. Después de que el recubrimiento 110 se despliegue completamente (no mostrado), el dispositivo de despliegue 116 puede retraerse y el instrumento quirúrgico 111 puede retirarse de un sitio quirúrgico.
Debe observarse que el extremo distal 110a del recubrimiento 110 puede asegurarse al conjunto de herramienta 152
o a un dispositivo de anastomosis extremo a extremo de funcionamiento similar o igual que no requiera agujeros tales como “H” (figura 12) para purgar directamente a la atmósfera.
Las figuras 14 a 16 ilustran otra forma de realización preferida del recubrimiento 210 del conjunto de herramienta y del dispositivo de despliegue de recubrimiento 214 incluyendo un manguito 216 posicionado alrededor de una parte de cuerpo 258 de una grapadora lineal 211 de anastomosis gastrointestinal endoscópica. Dicha grapadora se describe con detalle en la patente US nº 6.241.139, concedida el 5 de junio de 2001. El dispositivo de despliegue de recubrimiento 216 es sustancialmente idéntico en estructura y uso al dispositivo de despliegue descrito en relación con las figuras 1 a 10 y no se le describirá en la presente memoria con más detalle. El recubrimiento 210 es sustancialmente idéntico al recubrimiento 10 descrito anteriormente, pero incluye además una banda elástica 226 formada alrededor, dentro o integralmente con una parte central del recubrimiento 210. El recubrimiento 210 se despliega de una manera sustancialmente idéntica a la descrita anteriormente. Sin embargo, como se muestra en la figura 16, cuando la banda elástica 226 se fuerza sobre el extremo distal del conjunto de herramienta 252 del instrumento quirúrgico 211, la banda elástica 226 se contrae para cerrar el recubrimiento 210 alrededor del conjunto de herramienta 252. La bolsa encerrada impide que tejido y/o fluidos corporales se escapen del recubrimiento 210 e infecten el tejido sano en la cavidad corporal o en la incisión. Se observa que pueden emplearse una o más bandas elásticas 226 con cualquiera de los recubrimientos descritos en la presente memoria o que los mismos pueden incorporarse en ellas.
Las figuras 17 a 19 ilustran el recubrimiento 10 del conjunto de herramienta y el dispositivo de despliegue de recubrimiento 14 mostrados en las figuras 1-6 posicionados alrededor del cuerpo 358 de un instrumento disector ultrasónico 311. Dicho instrumento se describe en la patente US nº 6.024.750, que se expidió el 14 de febrero de 2000. Como se ilustra, el recubrimiento 10 se sujeta preferentemente al extremo distal del cuerpo alargado 358 del instrumento 311 proximalmente de un conjunto de herramienta 352 del instrumento 311 de tal manera que, cuando se despliega el recubrimiento 10 de la forma expuesta anteriormente con respecto a las figuras 1-6, el conjunto de herramienta 352 y/o el tejido contaminado o tratado quedan rodeados por el recubrimiento 10 (figura 19).
Aunque la invención se ha mostrado y descrito particularmente con referencia a las formas de realización preferidas, se entenderá por los expertos en la materia que pueden realizarse en ellas diversas modificaciones y cambios de forma y de detalle. Por ejemplo, puede proporcionarse un dispositivo de cierre diferente de una banda elástica para cerrar el recubrimiento alrededor del conjunto de herramienta. Por ejemplo, puede proporcionarse un cordón de tracción “D” (figura 10) que puede agarrarse en el sitio quirúrgico utilizando prensores para cerrar el recubrimiento 10
o el cordón de tracción puede alimentarse a través del instrumento quirúrgico y ser accesible desde la parte proximal del instrumento quirúrgico. Además, el recubrimiento no necesita invertirse alrededor del conjunto de herramienta sino que puede deslizarse directamente sobre el conjunto de herramienta. Asimismo, el recubrimiento puede configurarse no sólo para abarcar el conjunto de herramienta, sino que puede configurarse también para abarcar una parte distal o la parte de cuerpo completa del instrumento quirúrgico.
Claims (18)
- REIVINDICACIONES1. Instrumento quirúrgico (11) que comprende:una parte de cuerpo (58);un elemento de herramienta (52) soportado sobre el extremo distal de la parte de cuerpo; yun recubrimiento alargado (10) soportado sobre el instrumento en una primera posición en la que el elemento de herramienta está descubierto, pudiendo desplazarse el recubrimiento desde la primera posición hasta una segunda posición que rodea por lo menos parcialmente al elemento de herramienta;un dispositivo de despliegue de recubrimiento (14, 116) asociado funcionalmente con el recubrimiento, pudiendo desplazarse el dispositivo de despliegue de recubrimiento para desplazar el recubrimiento desde la primera posición hasta la segunda posición;en el que el recubrimiento incluye un extremo proximal y un extremo distal y define un lumen (12), estando sujeto el extremo distal del recubrimiento al instrumento quirúrgico sobre el elemento de herramienta o junto a éste de tal manera que el lumen del recubrimiento se posicione alrededor de la parte de cuerpo y del dispositivo de despliegue de recubrimiento, invirtiendo el movimiento del dispositivo de despliegue de recubrimiento desde una posición retraída hasta una posición avanzada invierte el recubrimiento.

- Instrumento quirúrgico según la reivindicación 1, en el que el recubrimiento es impermeable a los líquidos.

- Instrumento quirúrgico según las reivindicaciones 1 ó 2, en el que una parte (60) del recubrimiento está fijada al instrumento.

- Instrumento quirúrgico según las reivindicaciones 1, 2 ó 3, en el que el recubrimiento es tubular y presenta un extremo proximal y un extremo distal (10a), y en el que el recubrimiento está abierto en los extremos proximal y distal.

- Instrumento quirúrgico según cualquiera de las reivindicaciones anteriores, en el que el dispositivo de despliegue de recubrimiento incluye un manguito posicionado de manera deslizante alrededor de la parte de cuerpo entre la posición retraída y la posición avanzada, pudiendo deslizarse el manguito desde la posición retraída hasta la posición avanzada para mover el recubrimiento desde la primera posición hasta la segunda posición.

- Instrumento quirúrgico según la reivindicación 5, en el que el manguito incluye una primera y segunda semisecciones (16a, 16b), manteniéndose normalmente la primera y segunda semisecciones apoyadas a tope entre sí por medio de por lo menos un elemento expandible (34), siendo expandible el elemento expandible para permitir que las primera y segunda semisecciones se muevan hacia fuera una con respecto a otra.

- Instrumento quirúrgico según la reivindicación 6, en el que el elemento expandible es una junta tórica elástica.

- Instrumento quirúrgico según las reivindicaciones 5, 6 ó 7, en el que el manguito incluye una junta anular proximalmente situada (24) dimensionado para facilitar el movimiento del manguito entre las posiciones retraída y avanzada.

- 9.
- Instrumento quirúrgico según la reivindicación 6 ó 7 u 8 cuando están subordinadas a la reivindicación 6, en el que la primera semisección de manguito (16a) incluye por lo menos un saliente (18) y la segunda semisección de manguito (16b) incluye por lo menos una ranura (20), pudiendo desplazarse dicho por lo menos un saliente hacia dentro de dicha por lo menos una ranura para mantener la alineación entre la primera y segunda semisecciones cuando estas semisecciones se mueven hacia fuera una con respecto a otra.

- Instrumento quirúrgico según cualquiera de las reivindicaciones anteriores, en el que el dispositivo de despliegue de recubrimiento incluye un elemento de acoplamiento distal (30, 126), una parte de guiado proximal (24, 124) y una parte de cuerpo central (122) que interconecta el elemento de acoplamiento y la parte de guiado, estando soportado el dispositivo de despliegue de recubrimiento sobre la parte de cuerpo para permitir que se desplace el dispositivo de despliegue de recubrimiento con el fin de desplazar el recubrimiento hasta la segunda posición.

- Instrumento quirúrgico según cualquiera de las reivindicaciones anteriores, en el que el recubrimiento incluye un elemento tubular que define el lumen, estando sujeto el extremo distal del recubrimiento al instrumento, pudiendo desplazarse el extremo proximal del recubrimiento sobre el elemento de herramienta para rodear por lo menos parcialmente el conjunto de herramienta.

- Instrumento quirúrgico según la reivindicación 11, que incluye además un dispositivo de cierre (226, D) para cerrar el extremo proximal del recubrimiento después de que la misma se haya movido sobre el elemento de
herramienta. -
- 13.
- Instrumento quirúrgico según la reivindicación 12, en el que el dispositivo de cierre incluye una banda elástica
soportada por el recubrimiento. 5 -

- Instrumento quirúrgico según la reivindicación 12, en el que el dispositivo de cierre incluye un cordón de tracción.

- Instrumento quirúrgico según cualquiera de las reivindicaciones anteriores, en el que el extremo distal del
recubrimiento se sujeta de forma amovible al instrumento quirúrgico. 10 - 16. Instrumento quirúrgico según cualquiera de las reivindicaciones anteriores, en el que el instrumento quirúrgico es una grapadora circular (52).
- 17. Instrumento quirúrgico según cualquiera de las reivindicaciones 1 a 15, en el que el instrumento quirúrgico es un 15 disector ultrasónico.
- 18. Instrumento quirúrgico según cualquiera de las reivindicaciones 1 a 15, en el que el instrumento quirúrgico es una grapadora lineal (252).
