When viewing or printing in 100% zoom, it's not accurate.
Viewing: I know there have been discussions about how to get 100% zoom to work right; it should work right out of the box without having to edit advanced settings. On other PDF readers (Foxit, Acrobat), 100% is actually 100%. That is, you can hold a paper up to the screen and it's the same size.
Printing: When I generated a graphic that's 2" x 3" (using Kicad) and tried to view it at 100% zoom to see whether it's the right size (and failed), I went ahead and printed it. It printed out about 15/16th size. Not noticeable for most things, absolutely unusable for PCB prints. Note that I opened the same exact PDF in Foxit and Acrobat Reader, and they both printed the object the right size. Note I had printer settings set to 100% (no scaling or reducing to page size) for everything.
Attaching the PDF in question - the outline box should be 2" x 3".
