[Bug]: Fix typo for examples in create_plan.md

I noticed here [create_plan](https://github.com/humanlayer/humanlayer/blob/50e204aa1ba7e8494205ad26e359c9613121605e/.claude/commands/create_plan.md#L438)

the line should say `/create_plan` instead of `/implementation_plan` ?
