Description
This PR improves the UX when adding columns.
Previously a new 'blank' column line would be added when the type of the last column was changed away from the default. This was confusing and encouraged making columns with no name.
This PR changes so both the name and type must be filled in,

Solution and Design
We now perform the action-specific reducer, and then validate the columns, adding a blank one if needed.

OLD WAY: you now get a second column
NEW WAY: you have to add the type as well.
Limitations, known bugs & workarounds
The code is poorly structured, this is just the bug fix so I can move on to general cleanup.
