Please use this reporting form to provide detailed information about the incident you believe may be a possible Academic Integrity Policy violation.
When a report is received, our staff will review it, decide what an appropriate pathway to take to address the alleged misconduct. As a result, OACC may consult with you to see if the incident warrants our meeting with the student to address the alleged misconduct. We will investigate and take appropriate action. You and any witnesses you identify in the report may be contacted by OACC staff for details about the incident during the investigation. Please note that, university student conduct records are protected by the federal Family Educational Rights and Privacy Act. As such, information gathered during, and the results of any investigation will be reported only to the student in question and school officials with a legitimate educational interest (i.e., the reporting faculty member and other relevant school officials).
