Most often, you want to search in a language, not a region, so it's always recommended to specify the language. You only need to add the region if you want to explicitly restrict your search to a region.

SearXNG is an aggregator; not all engines support all selectors (filters), so you always end up with a mix of results. The dividing line between region and language is therefore not always clear, but the trend is always noticeable: for example compare the result lists of the following queries:

• bmw :en-US
• bmw :en-CA

Thanks for the detailed explanation @return42 and it makes a lot of sense. It's been a minute since I originally set up my instance but I can only imagine that I selected en-US out of habit like when selecting a keyboard language when it reality, 99.9% of my searches really are for general english results and not that I want english results for the US only.
